Position Summary:

Provide personal care, meal preparation, light housekeeping, and/or shopping and errands to patients in their homes as outlines by the patient’s Plan of Care. All care/tasks provided are as authorized by the State as acceptable for the level of care in which the independent contractor is functioning and must be detailed on the patient’s Plan of Care.

Essential Duties & Responsibilities for Position:

  1. Assist patients with personal care needs such as bathing, elimination needs, transferring, dressing/undressign, and ambulation together with any household task (e.g. making bed, changing linens, laundry, light housekeeping, etc.) essential to the patient’s health in accordance with teh patients individualized care plan.
  2. Plan/prepare nutritious meals and assist in feeding patients as directed.
  3. Document services provided in an accurate and timely manner in accordance with agency policy and personal care plan. Maintain patient confidentiality at all times.
  4. Measure and record patient’s temperature, pulse, respiratory rate and blood pressure when requested and/or directed by the Nursing staff and patient care plan.
  5. Report any changes in the patient’s condition or incidents to nursing staff in a timely manner.
  6. Perform tasks/skills as outlined on care plan competently.
  7. Notify Nursing staff when clarfication or education of a task is required.
  8. Submit timecards/activity sheets in a timely manner.
  9. Implements appropriate infection control practices.
  10. Demonstraties appropriate judgement skills to be able to make independent decisions in routine patient care matters.
  11. Assist patient with Range of Motion exercises when requrested or in accordance with patients Plan of Care.

  13. Attends and participates in all required staff meetings.
  14. Organizes workload to complete responsibilites in an appropriate and timely manner.
  15. Observe infection control, blood borne pathogens, and standard precautions.

Education and Qualification Requirements:

  1. Must be at least 18 years of age.
  2. Completion of training and competency program as prescribed by Regulatory Agencies of the State in which employed and as required by C.B. Helphing Hands and shall possess written evidence of such completion.

Personal and Physical Requirements:

  1. Pass a physical examination after being offered employment and a health assessment annually thereafter.
  2. Pass employment drug screen and random screening during employment.
  3. Possess reliable means of transportation to and from work.
  4. Able to life up to 50 pounds, perform physical requirements as needed to specific assignments.
  5. Able to withstand exposure to different allefgens.
  6. Endure exposure to weather and temperature extremes.
